This project aimed to address issues of duplicate records and low quality external data within GP portals and the Orion Health system. The creation of a new database and integration with APIs, the project linked related multiple concurrent patient diagnosis.
Solution: established a new data base and integrated it with APIs for seamless linking of diagnosis. In addition, the implementing user friendly filtering and management tools to enhance data organisation and accessibility. User experience surveys were conducted to gather feedback and refine the interface.

Users can efficiently navigated and access detailed patient medication data, reducing duplicate records and improving patient experience outcomes.
